Background: A preoperative marker for morbidity in patients with colorectal cancer would help to risk stratify patients and allow for timely intervention to avert poor outcomes. We conducted this study to evaluate preoperative lymphocyte-white blood cell ratio (LWR) as a marker of postoperative morbidity. Methods: A prospective cohort of patients who underwent elective surgery for colorectal cancer was reviewed. Three morbidity-related outcomes were described-overall morbidity, multiple morbidities, and severe morbidity, defined as Clavien-Dindo Class >= 3. Univariable and multivariable analyses of presurgical predictors of these three outcomes were performed. Preoperative variables included hemoglobin levels, neoadjuvant therapy, albumin levels, white blood cell count, lymphocyte count, LWR, neutrophil-lymphocyte ratio, and prognostic nutritional index. Results: Of 177 patients, 31.6% (56/177) suffered at least one morbidity, 15.3% (27/177) had multiple morbidities, 7.9% (14/177) suffered severe morbidity. On multivariate analysis, only LWR <0.180 (odds ratio [OR] 2.53, 95% confidence interval [CI] 1.15-5.55) and neoadjuvant therapy (OR 2.49, 95% CI 1.16-5.24) were associated with overall morbidity. For multiple morbidities and severe morbidity, only LWR <0.180 was significantly associated on multivariate analysis with an OR of 2.92 (95% CI 1.19-7.13) and 4.62 (95% CI 1.45-14.73), respectively. Conclusions: LWR is a preoperative marker which can be conveniently applied using standard preoperative blood tests. Triptolide (TP) is one of the most common systemic treatments for inflammatory and immune diseases in China for centuries. However, TP exhibits some disadvantages, such as poor solubility in water, poor bioavailability, liver toxicity, renal toxicity, and other side effects. In order to reduce the adverse effects of TP, researchers have developed numerous strategies to address the adverse properties of triptolide. Nano-carrier-based triptolide delivery systems represent an emerging technology and are one of the strategies of nanomedicine that combines diagnostic and therapeutic applications in a single agent. In this approach, we developed a glutathione-activated carrier-free nanodrug of triptolide (CyssTPN) as a trackable drug delivery system. In this system, CyssTP self-assemble to form a carrier-free nanodrug, which possesses a monodisperse spherical morphology with hydrodynamic average sizes of about 50 nm. In addition, CyssTPN had good stability under different physiological conditions (pH, high salt, etc.). Apart from cellular imaging and cell uptake, CyssTPN can be tracked by the activation of TP ability in real-time and applied for cancer cell treatment efficiently. The result showed that CyssTPN could improve solubility, reduce the side effects, and increase the bioavailability of triptolide. It could also track triptolide activation timely and tumor therapy successfully.

A triply responsive amphiphilic polymer containing poly (ethylene glycol) (PEG) and pyrene linked by a disulfide bond (PEO-SS-PY) was synthesized. Benefiting from its amphiphilicity, PEO-SS-PY could self-assemble into 126 nm micelles in water. PEO-SS-PY with CB[7] forms a 1:1 pseudorotaxane polymer (PEO-SS-Py@CB[7]) in aqueous solution. After the formation of host-guest complex, the micelles size increased to 136 nm and the fluorescence emission intensity increased. Glutathione would break the disulfide bond, leading to the dissociation of the micelles. Spermine and adamantanamine hydrochloride would dissociate the host-guest complex, leading to change of micelle size and decrease of fluorescence intensity. More interestingly, this self-assembly micelle displayed highly sensitive response to Fe3+. Because the effective absorption of Fe3+ hindered the absorption of PEO-SS-Py@CB[7] at the excitation wavelength of 405 nm, Fe3+ distinctly quenched the fluorescence emission of PEO-SS-Py@CB[7], whereas, a very low changes of intensity was observed for other metal ions. Results of this paper provide a new idea for the combination of metal ion detection and controlled release.

PURPOSE This report describes outcomes of an ongoing quality-improvement project (VitalSign6) in a large US metropolitan area to improve recognition, treatment, and outcomes of depressed patients in 16 primary care clinics (6 charity clinics, 6 federally qualified health care centers, 2 private clinics serving low-income populations, and 2 private clinics serving patients with either Medicare or private insurance). METHODS Inclusion in this retrospective analysis was restricted to the first 25,000 patients (aged >= 12 years) screened with the 2-item Patient Health Questionnaire (PHQ-2) in the aforementioned quality-improvement project. Further evaluations with self-reports and clinician assessments were recorded for those with positive screen (PHQ-2 >2). Data collected from August 2014 though November 2016 were available at 3 levels: (1) initial PHQ-2 (n = 25,000), (2) positive screen (n = 4,325), and (3) clinician-diagnosed depressive disorder with 18 or more weeks of enrollment (n = 2,160). RESULTS Overall, 17.3% (4,325/25,000) of patients screened positive for depression. Of positive screens, 56.1% (2,426/4,325) had clinician-diagnosed depressive disorder. Of those enrolled for 18 or more weeks, 64.8% were started on measurement-based pharmacotherapy and 8.9% referred externally. Of the 1,400 patients started on pharmacotherapy, 45.5%, 30.2%, 12.6%, and 11.6% had 0, 1, 2, and 3 or more follow-up visits, respectively. Remission rates were 20.3% (86/423), 31.6% (56/177), and 41.7% (68/163) for those with 1, 2, and 3 or more follow-up visits, respectively. Baseline characteristics associated with higher attrition were: non-white, positive drug-abuse screen, lower depression/anxiety symptom severity, and younger age. CONCLUSION Although remission rates are high in those with 3 or more followup visits after routine screening and treatment of depression, attrition from care is a significant issue adversely affecting outcomes.

Biomimetic actuators have seemingly infinite potential for use in previously unexplored areas. However, large stresses and a rapid water response are difficult to realize in soft actuators, owing to which their practical applicability is currently limited. In this paper, a new method for designing and fabricating humidity-responsive sturdy hard actuator. By combining a rigid matrix and hydrophilic water domains consisting of dynamic interlocking cation-? blocks, high-performance polymer actuator was synthesized that swell rapidly in response to a water gradient in their environment, resulting in unprecedentedly large stresses. More critically, the strong interlocking cation-? blocks reform and the intermolecular distance is reduced when the water is removed, allowing the deformed actuator to revert its original shape. The proposed design principle can potentially be extended to produce different types of sturdy actuators with rapid water responsiveness.

To achieve a high stability in physiological environment and rapid intracellular drug release, a biodegradable zwitterionic triblock copolymer with a disulfide-linked poly-epsilon-caprolactone and polycarboxybetaine methacrylate (PCBMA-SS-PCL-SS-PCBMA) was prepared for micellar carrier to delivery doxorubicin (DOX) into tumor cells. Self-assembling from PCBMA-SS-PCL-SS-PCBMA, polymeric micelles had many advantages, such as ultra-low protein absorption in serum and obvious reduction-responsiveness in the presence of DTT. Furthermore, DOX-loaded micelles exhibited high stability upon centrifugation and lyophilization, a fast intracellular drug release and enhanced drug efficacy due to GSH-triggered PCBMA shell shedding and micellar reassembling. Thus, the polymeric micelles integrated several functions and properties could be prospectively utilized as valuable nanocarriers in cancer chemotherapeutics.

Nitrogen-doped MoS2 quantum dots (N-MoS2 QDs) were synthesized via a facile hydrothermal approach, and exhibited high fluorescence quantum yield (QY, 14.9%), excellent photostability, biocompatibility and water solubility. 5-Carboxylcytosine (5caC) plays a vital role in the dynamics of DNA demethylation, and sequencing of its sites will help us dig out more biological functions of 5caC. Herein, we present a novel chemical method to efficiently label 5caC distinguished from other bases in DNA. Combined with bisulfite sequencing, 5caC sites can be located at single-base resolution, and the efficiency of 5caC labeling is 92% based on the Sanger sequencing data. Furthermore, dot blot assays have confirmed that 5caC-containing DNA isolated from HeLa cells was successfully labeled using our method. We expect that our strategy can be further applied to selectively tagging other carboxyl-modified bases and mapping their sites in RNA.
