Antimicrobial efficacy of photodynamic therapy on dental implant surfaces: A systematic review of in vitro studies

Background: To systematically review the literature regarding the antimicrobial effects of photodynamic therapy (PDT) on multi-bacterial species and the possible surface alterations of dental implants as a result of PDT. Methods: The addressed focused question was: ?Does PDT show antimicrobial efficacy against multi-bacterial species colonization and result in surface alteration on dental implants?? Electronic databases including MEDLINE and EMBASE up to and including December 2018 were searched. Results: Seven studies were included. Two studies used a total of 110 titanium dental implants, while 1 study included a total of 72 zirconia dental implants. Three studies investigated the antimicrobial PDT effects on titanium discs, while 1 study used titanium plates with germanium prisms. All in-vitro studies used diode laser. Energy fluence was reported only in 2 studies. Power output and density were 100 mW (mW) and 150 mW cm?2, respectively. All in-vitro studies reported the multibacterial species outcomes after the application of antimicrobial PDT. All studies showed a significant reduction in the bacterial load. Only two studies reported the outcomes of microstructural changes on the titanium surface, in which both studies did not report any significant alterations on the titanium implants or discs with the application of PDT. Conclusion: This systematic review demonstrated significant reduction in the bacterial load but inconclusive findings regarding structural alterations on the titanium surface with the use of PDT. The results of this review should be considered preliminary and further in-vitro studies with standardized laser parameters are needed to obtain strong conclusions.

Bactericidal efficacy of photodynamic therapy against Enterococcus faecalis in infected root canals: A systematic literature review

Objective: The aim was to review the bactericidal efficacy of photodynamic therapy (PDT) against Enterococcus faecalis (E. faecalis) in infected root canals. Methods: To address the focused question “Does PDT exhibit bactericidal effects against E. faecalis in infected root canals?” PubMed/Medline and Google-Scholar databases were searched from 1985 up to August 2013 using various combinations of the following key words: “antibacterial; “bactericidal; “endodontic; “root canal” and “photodynamic therapy”. Original studies, experimental studies and articles published only in English language were included. Letters to the editor, historic reviews and unpublished data were excluded. The pattern of the present review was customized to primarily summarize the pertinent information. Results: Seventeen studies (16 ex vivo and one in vivo) were included. In these studies, numbers of teeth used ranged between 30 and 220 teeth. In these studies, wavelengths of diode laser used, diameter of fiber and power output ranged between 625 and 805. nm, 200. mum and 0.4. cm, and 40. mW and 5. W respectively. Twelve studies reported PDT to be effective in eliminating E. faecalis from infected root canals. Four studies reported conventional irrigation and instrumentation to be more efficient in killing E. faecalis than PDT. One study reported PDT and conventional endodontic regimes to be equally effective in eliminating E. faecalis from infected root canals. In most studies, toluidine blue and/or methylene blue were used as photosensitizers. Conclusion: Efficacy of PDT in eliminating E. faecalis from infected root canals remains questionable.

Antimicrobial efficacy of photodynamic therapy and light-activated disinfection against bacterial species on titanium dental implants

Purpose: The aim of this study was to evaluate the efficacy of photodynamic therapy (PDT) and light-activated disinfection (LAD) against a 3-day-old bacterial suspension prepared from three different bacterial species present on titanium dental implants, and to analyze the possible alterations of the implant surfaces as a result of the PDT and LAD. Materials and Methods: The study was conducted on 72 titanium dental implants contaminated with a bacterial suspension prepared from three bacterial species: Prevotella intermedia, Aggregatibacter actinomycetemcomitans, and Porphyromonas gingivalis. The contaminated implants were incubated under anaerobic conditions for 72 hours and then were randomly divided into four experimental groups and two control groups (n = 12 each), according to the following treatment protocols: group 1 (PDT1): PDT (660 nm, 100 mW, 60 seconds) with toluidine blue; group 2 (PDT2): PDT (660 nm, 100 mW, 60 seconds) with phenothiazine chloride dye; group 3 (LAD): light-emitting diode (LED) with toluidine blue; group 4 (toluidine blue): treatment with only toluidine blue for 60 seconds. In the positive control group, the implants were treated with a 0.2% chlorhexidine-based solution for 60 seconds, and in the negative control group, no treatment was used. Results: The highest bacterial reduction was recorded in the PDT1 (98.3%) and PDT2 (97.8%) groups. The results of this study showed that there was a statistically significant reduction of bacteria in the PDT1 and PDT2 groups compared with the negative control group (P < .05), individually for each bacterial species as well as for all three species together. LAD was less effective than PDT1 and PDT2, and did not show a statistically significant difference compared with the negative control or any other treatment group. Toluidine blue was the least effective treatment in terms of both the total bacterial count and the individual count for each bacterial species. Conclusion: Both PDT1 and PDT2 protocols showed a high efficacy against a 3-day-old bacterial biofilm on dental implants and were more effective compared with LAD. Do you like my blog? The effects of repeated applications of antimicrobial photodynamic therapy in the treatment of residual periodontal pockets: a systematic review

The aim of this study was to assess the effects of repeated applications of antimicrobial photodynamic therapy (aPDT) on the non-surgical periodontal treatment of residual pockets. This work was performed and reported according to the Cochrane and PRISMA recommendations, respectively, and registered at the PROSPERO registry (number CRD42017058403). An extensive search of the biomedical literature was conducted on four databases from January 1960 to August 2018, followed by hand searching. Analysis of the quality of the selected studies was based on the risk of bias. Only two randomised controlled clinical trials (RCTs) met the inclusion criteria although they had unclear risk of bias. One study showed that repeated applications of aPDT in association with conventional non-surgical treatment during periodontal maintenance improved all clinical outcomes after 6?months. The other study, which assessed the effects of repeated applications of aPDT in association with ultrasound debridement on periodontal pathogens, showed no significant reduction of the main pathogens after 3?6?months but reported reductions of probing pocket depth and C-reactive protein after 3 and 6?months, respectively, compared to mechanical therapy alone. Concluding, it was not possible to state that repeated applications of aPDT, in association with non-surgical treatment of residual pockets, have effective clinical effects in the periodontal maintenance therapy. Although one can consider that aPDT is a promising adjuvant therapy, it is still necessary to carry out more RCTs with low risk of bias in order to confirm or refute the benefits of multiple applications for residual periodontal pockets.

Is adjunctive photodynamic therapy more effective than scaling and root planing alone in the treatment of periodontal disease in hyperglycemic patients? A systematic review

Background: To assess the impact of scaling and root planing (SRP) with and without adjunct photodynamic therapy (PDT) in the treatment of periodontal disease (PD) in hyperglycemic patients. Methods: Databases (MEDLINE, EMBASE; and CENTRAL) were searched up to December 2017. The addressed PICO question was: ?What is the effectiveness of adjunctive PDT to non-surgical periodontal treatment by means of clinical periodontal and glycemic parameters in hyperglycemic patients?? Results: Four clinical trials and 1 experimental study were included. Energy fluence, power output, power density and duration of irradiation were 2.79 J per square centimeters (J cm?2), 150 milliwatts (mW), 428 milliwatts per square centimeters (mW/cm2) and 133 s (s) respectively. All studies reporting clinical periodontal and metabolic parameters, showed that aPDT was effective in the treatment of periodontal inflammation in hyperglycemic patients at follow-up. When compared with SRP alone, none of the studies showed additional benefits of PDT as compared to SRP alone at follow up. Three studies showed no influence of SRP with or without aPDT on HbA1c levels. One study showed a significant reduction of HbA1c levels in adjunctive aPDT as compared to SRP alone at follow-up. Conclusion: It remains debatable whether adjunctive PDT as compared to SRP is effective in the treatment of periodontal inflammation and reduction of HbA1c levels in hyperglycemic patients.
