Effect of non-surgical periodontal therapy on glycemic control of type 2 diabetes mellitus: A systematic review and Bayesian network meta-analysis

Background: Glycemic control is vital in the care of type 2 diabetes mellitus (T2DM) and is significantly associated with the incidence of clinical complications. This Bayesian network analysis was conducted with an aim of evaluating the efficacy of scaling and root planning (SRP) and SRP + adjuvant treatments in improving glycemic control in chronic periodontitis (CP) and T2DM patients, and to guide clinical practice. Methods: We searched the Pubmed, Embase, Cochrane Library and Web of Science databases up to 4 May 2018 for randomized controlled trials (RCTs). This was at least three months of the duration of study that involved patients with periodontitis and T2DM without other systemic diseases given SRP. Patients in the control group did not receive treatment or SRP combination with adjuvant therapy. Outcomes were given as HbA1c% and levels fasting plasma glucose (FPG). Random-effects meta-analysis and Bayesian network meta-analysis were conducted to pool RCT data. Cochrane’s risk of bias tool was used to assess the risk of bias. Results: Fourteen RCTs were included. Most were unclear or with high risk of bias. Compared to patients who did not receive treatment, patients who received periodontal treatments showed improved HbA1c% level, including SRP (the mean difference (MD) -0.399 95% CrI 0.088 to 0.79), SRP + antibiotic (MD 0.62, 95% CrI 0.18 to 1.11), SRP + photodynamic therapy (aPDT) + doxycycline (Doxy) (MD 1.082 95% CrI 0.13 to 2.077) and SRP + laser (MD 0.66 95% CrI 0.1037, 1.33). Among the different treatments, SRP + aPDT + Doxy ranked best. Regarding fasting plasma glucose (FPG), SRP did not show advantage over no treatment (MD 4.91 95% CI – 1.95 to 11.78) and SRP with adjuvant treatments were not better than SRP alone (MD -0.28 95% CI -8.66, 8.11). Conclusion: The results of this meta-analysis seem to support that periodontal treatment with aPDT + Doxy possesses the best efficacy in lowering HbA1c% of non-smoking CP without severe T2DM complications. However, longer-term well-executed, multi-center trails are required to corroborate the results.

Bactericidal efficacy of photodynamic therapy against Enterococcus faecalis in infected root canals: A systematic literature review

Objective: The aim was to review the bactericidal efficacy of photodynamic therapy (PDT) against Enterococcus faecalis (E. faecalis) in infected root canals. Methods: To address the focused question “Does PDT exhibit bactericidal effects against E. faecalis in infected root canals?” PubMed/Medline and Google-Scholar databases were searched from 1985 up to August 2013 using various combinations of the following key words: “antibacterial; “bactericidal; “endodontic; “root canal” and “photodynamic therapy”. Original studies, experimental studies and articles published only in English language were included. Letters to the editor, historic reviews and unpublished data were excluded. The pattern of the present review was customized to primarily summarize the pertinent information. Results: Seventeen studies (16 ex vivo and one in vivo) were included. In these studies, numbers of teeth used ranged between 30 and 220 teeth. In these studies, wavelengths of diode laser used, diameter of fiber and power output ranged between 625 and 805. nm, 200. mum and 0.4. cm, and 40. mW and 5. W respectively. Twelve studies reported PDT to be effective in eliminating E. faecalis from infected root canals. Four studies reported conventional irrigation and instrumentation to be more efficient in killing E. faecalis than PDT. One study reported PDT and conventional endodontic regimes to be equally effective in eliminating E. faecalis from infected root canals. In most studies, toluidine blue and/or methylene blue were used as photosensitizers. Conclusion: Efficacy of PDT in eliminating E. faecalis from infected root canals remains questionable.

Learning from clinical phenotypes: Low-dose biophotonics therapies in oral diseases

This narrative review on the use of biophotonics therapies for management of oral diseases is written as a tribute to Prof. Crispian Scully. His seminal contributions to the field are highlighted by the detailed, comprehensive description of clinical presentations of oral diseases. This has enabled a more thorough, fundamental understanding of many of these pathologies by research from his group as well as inspired mechanistic investigations in many groups globally. In the same vein, a major emphasis of this narrative review is to focus on the evidence from human case reports rather than in vitro or in vivo animal studies that showcases the growing and broad impact of biophotonics therapies. The similarities and differences between two distinct forms of low-dose biophotonics treatments namely photodynamic therapy and photobiomodulation therapy are discussed. As evident in this review, a majority of these reports provide promising evidence for their clinical efficacy. However, a lack of adequate technical details, precise biological rationale, and limited outcome measures limits the current utility of these treatments. Future investigations should attempt to address these shortcomings and develop better designed, rigorous, controlled studies to fully harness the tremendous potential of low-dose biophotonics therapies.

Adjunctive use of antimicrobial photodynamic therapy in the treatment of medication-related osteonecrosis of the jaws: A case report

Medication-related osteonecrosis of the jaws is a delayed healing condition commonly recognized as a serious adverse effect of antiresorptive therapy and is particularly associated with aminobisphosphonate treatment. In the present report, a 62-year-old osteoporotic female patient treated with intramuscular injections of clodronate for 3 years exhibited clinical signs of exposed necrotic bone and inflamed mucosa 4 months after tooth extractions in the mandibular interforaminal region. The treatment consisted of sequestrectomy, open-flap debridement, and adjunctive use of antimicrobial photodynamic therapy to decontaminate the affected hard and soft tissues. Successful results were obtained and maintained from both clinical and radiological aspects. No recurrence was observed up to 6 months of follow-up.

Clinical efficacy of photodynamic therapy and laser irradiation as an adjunct to open flap debridement in the treatment of chronic periodontitis: A systematic review and meta-analysis

Background: The aim of this systematic review and meta-analyses was to assess the quality of evidence and efficacy of antimicrobial photodynamic therapy (aPDT) and laser irradiation (LI) as an adjunct to open flap debridement (OFD) in the treatment of chronic periodontitis. Methods: Electronic searches were conducted in databases (MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials and Cochrane Oral Health Group Trials Register databases) up to March 2019. Randomized clinical trials (RCTs) comparing clinical efficacy of either aPDT and/or LI, placebo, or no treatment were included. Primary outcomes included clinical attachment level (CAL), while secondary outcomes were reduction in probing depth (PD) and gingival recession (GR) depth. The weighted mean differences (WMD) of outcomes and 95% confidence intervals (CI) for each variable were calculated using random effect model. Results: Six RCTs were included. For aPDT studies, the overall mean difference for CAL gain (WMD?=??0.61, 95% CI?=??1.22 to ?0.016, P?=.044) and PD reduction (WMD?=??1.79, 95% CI?=??3.44 to ?0.14, P?=.034) was significant between aPDT and OFD groups at follow-up. No significant overall mean difference was observed for GR depth (WMD?=?0.02, 95% CI?=??0.75 to 0.79, P?=.95). For LI studies, none of the clinical periodontal parameters including CAL gain (WMD?=?0.23, 95% CI?=??0.09 to 0.55, P?=.159, Figure 3A), PD reduction (WMD?=?0.31, 95% CI?=??0.67 to 1.31, P?=.52, Figure 3B) and GR depth (WMD?=??0.34, 95% CI?=??2.47 to 1.78, P?=.74, Figure 3C) were found to be significant between LI and OFD groups at follow-up. Conclusion: With the limited data available, only aPDT as an adjunct to OFD showed superior results for clinical periodontal parameters compared to OFD alone in the treatment of chronic periodontitis. Further RCTs are warranted in order to obtain robust conclusions with regard to laser therapy.

Is Antimicrobial Photodynamic Therapy Effective for Microbial Load Reduction in Peri-implantitis Treatment? A Systematic Review and Meta-Analysis

The systematic review and meta-analysis were undertaken to evaluate the effectiveness of antimicrobial photodynamic therapy (aPDT) in the microbiological alteration beneficial to peri-implantitis treatment. This study is registered with PROSPERO, number CRD42017064215. Bibliographic databases including Cochrane Library, Web of Science, Scopus and PubMed were searched from inception to 8 January 2017. The search strategy was assembled from the following MeSH Terms: ?Photochemotherapy,? ?Dental Implants? and ?Peri-Implantitis.? Unspecific free-text words and related terms were also included. The Cochrane Collaboration’s tool was used to evaluate the risk of bias of included studies. The random-effect model was chosen, and heterogeneity was evaluated using the I2 test. Three studies met the inclusion criteria. Meta-analysis demonstrated an association between aPDT and reduction in viable bacteria counts for: Aggregatibacter actinomycetemcomitans (OR = 1.31; confidence interval = 1.13, 1.49; P < 0.00001), Porphyromonas gingivalis (OR = 4.08; confidence interval = 3.22, 4.94; P < 0.00001) and Prevotella intermedia (OR = 1.66; confidence interval = 1.06, 2.26; P < 0.00001). The weighted mean differences (WMD) of outcomes and 95% confidence intervals (CI) for each variable were calculated using random and fixed effect models. Results: A total of 5 RCTs were included. Two clinical trials were classified as having a low risk of bias and 3 clinical trials as high risk of bias. The overall effect for CAL gain (WMD= ?0.088, 95% CI= ?0.40 to 0.23, p = 0.58) and PD reduction (WMD= -0.35, 95% CI= -0.87 to -0.17, p = 0.18) was not statistically significant between PDT and SRP groups at follow-up, respectively. Conclusion: Within the limitations of this study, it remains debatable whether PDT as an adjunct to SRP is more effective in clinical attachment level gain as compared to SRP alone in cigarette smokers given that the available scientific evidence is weak.

Efficacy of photodynamic therapy as adjunct treatment of chronic periodontitis: a systematic review and meta-analysis

Meta-analysis of treatment effects of antimicrobial photodynamic therapy (aPDT) adjunct to non-surgical scaling and root planing (SRP) in comparison to SRP alone on patients with chronic periodontitis. The meta-analysis was performed according to PRISMA statement and Cochrane Collaboration guidelines. Electronic search complemented by hand search assured a high yield of randomized controlled trials (RCTs) of aPDT as adjunct modality to SRP. Differences in probing depth (PD) and clinical attachment level (CAL) were calculated with 95% confidence intervals and pooled in a random effects model. Analysis for intra- and inter-study heterogeneity was provided by chi2 and I2 tests, and publication bias was checked by funnel plots. Pooled overall effects of 26 RCTs attested significant benefits of aPDT adjunct to SRP with respect to PD reduction (MD 0.37; 95% CI 0.12?0.53; P < 0.0001) and CAL gain (MD 0.33; 95% CI 0.19?0.48; P < 0.00001) after 3 and 6?months. Sensitivity analysis minimized heterogeneity of PD reduction (MD 0.21; 95% CI 0.13?0.30; P < 0.00001) and CAL gain (MD 0.36; 95% CI 0.27?0.46). aPDT adjunct to SRP provides significant PD reduction and CAL gain in treatment of chronic periodontitis. The periodontal disease (PD) etiology is mainly associated with some bacterial strains, such as Porphyromonas gingivalis (P. gingivalis). Nonsurgical root scaling (e.g., antibiotics) may achieve a temporary decrease in the P. gingivalis level, yet it cannot eradicate the microorganism. Moreover, antibiotics can lead to bacterial resistance and undesirable side effects. This systematic review was performed to identify animal data defining antimicrobial photodynamic therapy (PACT) role on experimental PD models in the treatment of P. gingivalis. Embase, MEDLINE, and PubMed were examined for studies published from January 1980 to August 2018. MeSH terms and Scopus data were used to find more related keywords. Four studies were selected and reviewed by two independent researches with a structured tool for rating the research quality. The beneficial effect of PACT included reductions in P. gingivalis counts, bleeding on probing, redness, and inflammation on multiple sites (i.e., first molar, dental implants; subgingival; and mandibular premolars). Although our results suggest that PACT displays antimicrobial action on P. gingivalis, thus improving the PD, a nonuniformity in the PACT protocol and the limited number of studies included lead to consider that the bactericidal efficacy of PACT against periodontal pathogens in PD remains unclear.

Introduction: Effective decontamination of the root canal system is key to the success of endodontic treatment. Photodynamic Therapy (PDT) may be an important tool in the reduction of root canal pathogens. Objectives: This study aimed to review the literature on the use of this therapy in Endodontics, and which clinical protocol has been used. Methods: A literature search was carried out using the following databases: PubMed and Scielo, and the following key-words: endodontics; photodynamic therapy; photodynamic therapy endodontics; light-activated disinfection root canal; light activated disinfection endodontics; photo-activated disinfection endodontics; photo-activated disinfection. Results: We selected 18 articles which fit the inclusion criteria: original and relevant articles, clinical and laboratory research, in permanent human teeth, as well as articles reporting the use of photodynamic therapy as a single or adjunct treatment to root canal disinfection, in addition to its efficacy in bacterial reduction. Conclusions: PDT has been shown to be an effective method in microbial reduction of root canals; however, it should be used as an additional method to conventional treatment.
