Is adjunctive photodynamic therapy more effective than scaling and root planing alone in the treatment of periodontal disease in hyperglycemic patients? A systematic review

Background: To assess the impact of scaling and root planing (SRP) with and without adjunct photodynamic therapy (PDT) in the treatment of periodontal disease (PD) in hyperglycemic patients. Methods: Databases (MEDLINE, EMBASE; and CENTRAL) were searched up to December 2017. The addressed PICO question was: ?What is the effectiveness of adjunctive PDT to non-surgical periodontal treatment by means of clinical periodontal and glycemic parameters in hyperglycemic patients?? Results: Four clinical trials and 1 experimental study were included. Energy fluence, power output, power density and duration of irradiation were 2.79 J per square centimeters (J cm?2), 150 milliwatts (mW), 428 milliwatts per square centimeters (mW/cm2) and 133 s (s) respectively. All studies reporting clinical periodontal and metabolic parameters, showed that aPDT was effective in the treatment of periodontal inflammation in hyperglycemic patients at follow-up. When compared with SRP alone, none of the studies showed additional benefits of PDT as compared to SRP alone at follow up. Three studies showed no influence of SRP with or without aPDT on HbA1c levels. One study showed a significant reduction of HbA1c levels in adjunctive aPDT as compared to SRP alone at follow-up. Conclusion: It remains debatable whether adjunctive PDT as compared to SRP is effective in the treatment of periodontal inflammation and reduction of HbA1c levels in hyperglycemic patients.

1,2-Didehydrophenothiazines: Preparation of 1-Alkyl and 1-Aryl-substituted Phenothiazines by Lithium-directed Alkylation

A general route to 1-alkyl substituted phenothiazines is described.Reactions of 2-chlorophenothiazine with an excess of methyl-, n-butyl-, s-butyl-, t-butyl- or phenyl-lithiums give the corresponding 1-substituted phenothiazines via 10-lithio-1,2-dehydrophenothiazine.Attempts to trap the presumed 1,10-dilithio intermediate failed. The reaction of 1-chlorophenothiazine with s-butyl-lithium results in preferential metallation of the 9-position. 2-Chlorophenothiazine reacts with nitrogen nucleophiles (lithium dialkylamides) to give 2-substituted products.

Utilisation of antimicrobial photodynamic therapy as an adjunctive tool for open flap debridement in the management of chronic periodontitis: A randomized controlled clinical trial

Background: Antimicrobial photodynamic therapy (aPDT) has proved to be an effective adjunctive modality with potential benefits in the management of chronic periodontitis. The combination of photothermal and photodynamic effects of Indocyanine green (ICG) dye, when it is photoactivated with a diode laser of 810 nm wavelength, has been well documented in literature. Aim: This study was conducted to evaluate whether a single session of antimicrobial photodynamic therapy using ICG dye-810 nm diode laser combination can provide a substantial benefit when it is utilised as an adjunct to open flap debridement (OFD) in the management of chronic periodontitis. Materials and method: Following thorough scaling and root planing, a comparative split mouth randomized controlled clinical trial was carried out on 20 recruited subjects who provided one test (OFD + aPDT) and one control site (OFD alone) each (total 40 treatment sites). The test group was subjected to a single episode of aPDT using ICG photosensitiser dye (1 mg/ml) activated with 810 nm diode laser. The laser was used in a continuous wave, non-contact mode at a power output of 100 mW applied for 30 s/spot (the total of 4 spots per tooth) and delivered by 400 mum fibre, to provide a fluence (energy density) value of 0.0125 J/cm2 per spot and generate a total energy of 3 J. The following clinical parameters were assessed at baseline and 3 months: probing pocket depth (PPD), relative attachment level (RAL), relative gingival margin level (RGML), plaque index (PI), gingival index (GI), and gingival bleeding index (GBI). Intragroup and intergroup comparison was performed using paired t-test and independent samples t-test respectively. Results: Intragroup comparison revealed a statistically significant improvement from baseline visit (p < 0.05). Intergroup comparison showed a statistically significant improvement in RAL, RGML and GI in the test group (p < 0.05). Conclusion: Utilisation of ICG dye activated with 810 nm diode laser, which mediated aPDT, has demonstrated surplus clinical improvement following OFD in the management of chronic periodontitis. Is antimicrobial photodynamic therapy effective as an adjunct to scaling and root planing in patients with chronic periodontitis? A systematic review

The aim of this systematic review was to investigate whether antimicrobial photodynamic therapy (aPDT) as either a primary mode of treatment or an adjunct to non-surgical treatment was more effective than scaling and root planing (SRP) alone in treating chronic periodontitis in terms of clinical attachment level (CAL) gain and probing depth (PD) reduction. The focused question was developed using the Patient, Intervention, Comparison, and Outcome (PICO) format, and two authors independently searched the Medline, EMBASE, Cochrane Library, Web of Science, Google Scholar, and Scopus databases for relevant studies from January 2008 to December 2016. Twenty studies included in this systematic review were randomized clinical trials (RCTs) or quasi-RCTs of aPDT compared to placebo, no intervention, or non-surgical treatment in an adult population. Basic study characteristics, photosensitizing agents and wavelengths used in aPDT, frequency of aPDT application, effect of aPDT on clinical parameters, antimicrobial effect of aPDT in chronic periodontitis, effect of immunological parameters following aPDT and patient-based outcome measures were collected from the studies. Although there was a wide range of heterogeneity in the included studied, they all indicated that aPDT has the potential to be an effective adjunct in the treatment of chronic periodontitis. Long-term, multicenter studies with larger sample sizes are needed before aPDT can be recommended as an effective treatment modality.

Photodynamic therapy for the treatment of peri-implant diseases: A network meta-analysis of randomized controlled trials

Background Photodynamic therapy in peri-implantitis has been tested in randomized clinical trials. Though systematic review and meta-analysis on human and animal studies, identifies the utility of photodynamic therapy, a comparison of other interventions with photodynamic therapy for peri-implantitis does not exist. Hence the aim of this network meta-analysis is to identify the role of photodynamic therapy for peri-implantitis compared with other interventions tested in randomized human clinical trials. Method Randomized controlled trials comparing photodynamic therapy and other interventions in patients with peri- implantitis was searched for in electronic databases. The risk of bias was calculated using Cochrane risk of Bias tool. The heterogeneity between the studies in direct comparison was assessed using Chi-square and I2 tests. GRADE working group approach was used to assess the quality of evidence. Publication bias was assessed using Funnel plot and Trim and Fill method was used to identify the number of missing studies. Results We observed a significant reduction in the level of attachment scores with the use of combined photodynamic therapy with mechanical debridement when compared with other interventions tested. For bleeding on probing, pocket depth and plaque scores no statistically significant results were obtained. Conclusion The use of photodynamic therapy with mechanical debridement will definitely bring about significant improvement in patients with peri-implantitis. Further trials on the use of photodynamic therapy with other treatment modalities need to be tested to arrive at the best possible treatment option for peri-implantitis.

Antimicrobial efficacy of photodynamic therapy and light-activated disinfection against bacterial species on titanium dental implants

Purpose: The aim of this study was to evaluate the efficacy of photodynamic therapy (PDT) and light-activated disinfection (LAD) against a 3-day-old bacterial suspension prepared from three different bacterial species present on titanium dental implants, and to analyze the possible alterations of the implant surfaces as a result of the PDT and LAD. Materials and Methods: The study was conducted on 72 titanium dental implants contaminated with a bacterial suspension prepared from three bacterial species: Prevotella intermedia, Aggregatibacter actinomycetemcomitans, and Porphyromonas gingivalis. The contaminated implants were incubated under anaerobic conditions for 72 hours and then were randomly divided into four experimental groups and two control groups (n = 12 each), according to the following treatment protocols: group 1 (PDT1): PDT (660 nm, 100 mW, 60 seconds) with toluidine blue; group 2 (PDT2): PDT (660 nm, 100 mW, 60 seconds) with phenothiazine chloride dye; group 3 (LAD): light-emitting diode (LED) with toluidine blue; group 4 (toluidine blue): treatment with only toluidine blue for 60 seconds. In the positive control group, the implants were treated with a 0.2% chlorhexidine-based solution for 60 seconds, and in the negative control group, no treatment was used. Results: The highest bacterial reduction was recorded in the PDT1 (98.3%) and PDT2 (97.8%) groups. The results of this study showed that there was a statistically significant reduction of bacteria in the PDT1 and PDT2 groups compared with the negative control group (P < .05), individually for each bacterial species as well as for all three species together. LAD was less effective than PDT1 and PDT2, and did not show a statistically significant difference compared with the negative control or any other treatment group. Toluidine blue was the least effective treatment in terms of both the total bacterial count and the individual count for each bacterial species. Conclusion: Both PDT1 and PDT2 protocols showed a high efficacy against a 3-day-old bacterial biofilm on dental implants and were more effective compared with LAD. Do you like my blog? Antimicrobial efficacy of photodynamic therapy on dental implant surfaces: A systematic review of in vitro studies

Background: To systematically review the literature regarding the antimicrobial effects of photodynamic therapy (PDT) on multi-bacterial species and the possible surface alterations of dental implants as a result of PDT. Methods: The addressed focused question was: ?Does PDT show antimicrobial efficacy against multi-bacterial species colonization and result in surface alteration on dental implants?? Electronic databases including MEDLINE and EMBASE up to and including December 2018 were searched. Results: Seven studies were included. Two studies used a total of 110 titanium dental implants, while 1 study included a total of 72 zirconia dental implants. Three studies investigated the antimicrobial PDT effects on titanium discs, while 1 study used titanium plates with germanium prisms. All in-vitro studies used diode laser. Energy fluence was reported only in 2 studies. Power output and density were 100 mW (mW) and 150 mW cm?2, respectively. All in-vitro studies reported the multibacterial species outcomes after the application of antimicrobial PDT. All studies showed a significant reduction in the bacterial load. Only two studies reported the outcomes of microstructural changes on the titanium surface, in which both studies did not report any significant alterations on the titanium implants or discs with the application of PDT. Conclusion: This systematic review demonstrated significant reduction in the bacterial load but inconclusive findings regarding structural alterations on the titanium surface with the use of PDT. The results of this review should be considered preliminary and further in-vitro studies with standardized laser parameters are needed to obtain strong conclusions.

Studies on Diamagnetic Susceptibility of Biologically Active Heterocycles. 1. Diamagnetic Susceptibility of Phenothiazines

Diamagnetic susceptibilities for a number of phenothiazines are reported.A theoretical method to estimate the diamagnetic susceptibility of these biologically active heterocycles is presented.Phenothiazines have been considered to be composed of two units: one consisting of a benzene nucleus containing amino and thio groups at ortho positions to the substituents and the other consisting of a benzene ring with substituents. The diamagnetic contributions of these two units have been obtained from the diamagnetic susceptibilities of substituted o-aminobenzenethiols and benzenes and have been used in estimating the diamagnetic susceptibilities of phenothiazines. It has provided excellent theoretical results.Such an excellent agreement between measured and estimated values is due to the fact that the interactions between substituents which affect the diamagnetism have been duly accounted for in such calculations.

Mass Spectral Fragmentation Patterns of Heterocycles. VII. Reinvestigation of Fundamental Processes in Phenothiazines

Electron impact induced fragmentation patterns of simple phenothiazines have been reinvestigated using metastable ion studies, exact mass measuremants and deuterated derivatives.Secondary fragmentation processes involving ions m/e 198, 171, 167, 166, 154, 140 and 139 have been clarified.Mechanisms for the release of sulfur (SH. and CSH.) nitrogen (HCN and H2CN.) containing fragments from phenothiazine molecular ion are proposed based on the deuterium content of the daughter ions obtained from 1,9-dideuteriophenothiazine.A revised mechanism for the expulsion of ketene from 10-acetylphenothiazine is suggested based on the fragmentation pattern of the 1,9-dideuterioderivative.The composition of m/e 140 was determined by high resolution measurement to be C10H6N and not C11H7 as previously reported.
